New Patients
Please arrive 15 minutes prior to appointment time to fill out new patient information forms and go over office policy.
Please have the following ready for your appointment:
- Valid form of Identification of patient/guardian
- Drivers License, Passport, State Identification Card, Texas CHL, etc.
- Medical and vision insurance cards
- Physical or electronic copy
- List of any medications patient is taking
- Any old glasses and contact lens prescriptions

Existing Patients
The Patient Portal can be used to access existing prescriptions and communicate with the office.
